Complete the Vanderbilt Rating Scale about Your Child

If your child’s provider has requested that you complete an assessment of your child’s behavioral and emotional problems, you can complete the assessment online right here. The questions are included in a commonly used rating scale, called the Vanderbilt ADHD Diagnostic Parent Rating Scale (the "Vanderbilt"). It assesses problems in 4 areas: 1) attention deficits or hyperactivity, 2) oppositional/defiant behavior, 3) conduct problems, and 4) anxiety or depression. And it includes some questions about your child's activities. Each item or question is rated on a 4-point scale.

The information from these questions will help your provider better serve your child's behavioral needs.

Your results will not be recorded or save here to keep them confidential. So, please save or print a copy for your records, as outlined below.

It will take about 15 minutes to complete.

Instructions:

  1. Access questions below. Select the parent assessment.
  2. Select the best answer to each question
  3. Please print or take a screen shot of the results on the final page. This will print out the summary scores based on your answers, which you can give to your provider.
  4. Bring the completed printout or screen shot with the summary to your clinician.
